Q. What should I bring with me?
Please print and fill out the appropriate patient forms prior to your appointment. (We will let you know which one when you call for an appointment.) Bring any imaging you have had done previously, which includes MRI and X-ray images.
Q. How do I get a refill of my medication?
You must call your pharmacy and have them fax a request to our office. Our fax number is (801) 314-5112.
Q. Can I get MRI or lab results explained over the phone?
You must schedule an appointment to have Dr. Bowen, Dr. Hurst, or Dr. Clements explain your results.
Q. Will my insurance cover procedures done in this office?
This is something that varies depending on your provider. Is is your responsibility to contact your insurance provider to inquire about your coverage.
